Lost SSS Number Retrieval Guide (Philippine Context)

6.2. Walk-In or Assisted Retrieval
- Prepare IDs (per §5).

6.4. Employer or HR Verification
Under §24 (f), R.A. 11199 an employer must keep payroll records for 10 years and submit R-3 (Contribution Collection List). An employee may:
- Ask HR to print the Contribution Collection List entry or the R-5 return that shows the SS number; or
- Secure a Certificate of Employment quoting the SS number (accepted by SSS branches as secondary proof).
6.5. PhilSys-Enabled Retrieval (Pilot)
Since May 2024, members who presented a PhilSys National ID at enrollment can walk into designated SSS counters and retrieve their number through PhilSys API lookup plus fingerprint scan—no form needed.
