- Member’s Change of Information Form (MCIF, HQP-PFF-049): The official, duly accomplished form highlighting the current data vs. the new/corrected data.
- Valid Government-Issued ID: One original copy showing the correct information and photo (e.g., UMID, Passport, Driver’s License, PRC ID).

Correction of Name, Birth Date, or Birthplace: * PSA-issued Birth Certificate.

- Internal Conciliation: Pag-IBIG’s Employer Compliance Division will first verify the records and issue a demand letter to the employer to reconcile the accounts and pay any back-contributions along with penalties.
